Google Scholar can be a great search tool, but it's sometimes difficult to get the full text of the things you find there. If you use this link to Google Scholar, you will be able to see which of your results is available from the Delhi library, like this:

Click on the "Find Full Text at Delhi" links to be taken to the full text. Other links on that side of your search results generally indicate freely available full-text; look for those, too.

Under the main search box in CINAHL, look for an "Advanced Search" link. This will open up three search boxes instead of just one, like this:
Having more search boxes to work with can help you by separating the different aspects of your search. In the example above, this search would give you qualitative studies about providing aids/hiv care to teens. This advanced search should give better results than typing all the terms into one box.
